Case Manager RN – Hyrbid Contract NEW

Pride Healthcare is seeking a Case Manager!
This is a contract position

The responsibilities of the temp Case Manager will be completing comprehensive telephonic (remote) and in-person assessments including plan of care for assigned members and patients. The temp Case Manager will be working in a hybrid setting, where travel is required to members/patient’s homes and/or other settings, including community centers, hospitals, or providers’ offices.

Hybrid Position: Travel to appointments to perform patient assessments, and then return home to input information into the system.
Contract: 1 Year plus
Schedule: Monday – Friday 8AM to 5PM (1 hour break)
Mileage is reimbursed for travel. (Essex County, Hudson County, + Union County)

Qualifications:

  • Active, Unrestricted New Jersey RN License
  • BLS/CPR Certification
  • Minimum of 1 year of nursing experience
  • Proficiency with Microsoft Word, Excel, Outlook
  • Minimum of Associates of Nursing degree
